1인 1좌석만 선택 가능하며, 중복 선택은 불가합니다.
Only one seat per person can be selected, duplicate selections are not allowed.
신청 완료 후 좌석 변경은 불가능합니다.
Seat changes are not allowed after registration is completed.
타인에게 양도 및 거래는 불가함을 안내드립니다.
Transfers or transactions to others are not permitted.
본 신청서 접수 이후 1월 24일(수)에 보증금 결제 링크가 일괄 발송될 예정이며, 모바일 티켓은 보증금 결제까지 완료 후 발행됩니다.
After the receipt of this application, a deposit payment link will be sent collectively on January 24th (Wednesday), and mobile tickets will be issued after the deposit payment is completed.
현장에서 추가 본인확인 후 입장 팔찌를 증정하며 모바일 티켓과 신분증을 모두 가져오셔야 입장 팔찌로 교환받을 수 있습니다.
신분증 인정범위 : 성명, 사진, 생년월일이 기재된 신분증
(주민등록증, 여권, 외국인 등록증, 학생증 등)
At the venue, an admission wristband will be provided after additional identity verification. Please bring both the mobile ticket and identification card for the exchange of the admission ticket and wristband.
Accepted forms of identification: An identification card that includes your name, photo, and date of birth
(resident registration card, passport, alien registration card, student ID, etc.)

제공 목적 : 행사 이용에 따른 본인 확인 절차에 이용, 모바일 티켓 발송 및 위약금 환불 관련 안내

보유 기간 : 행사 종료 후 1개월 이내 파기

예약 보증금 제도
(reservation deposit system)
본 행사는 노쇼 방지를 위하여 예약 보증금 1만 원을 받고 있습니다.
보증금은 본 신청서 접수 이후 모바일 티켓 발급 시 카드로 결제할 예정이며, 예약 보증금 결제가 확인되지 않은 예약은 취소될 수 있습니다.
납입된 예약 보증금은 현장 참여 확인 후 행사 종료 후 7일 이내 전액 환불될 예정이며, 현장 미방문자의 예약 보증금은 환불되지 않습니다.
위와 같이 예약 보증금 제도 운영에 동의하십니까?
To prevent no-shows, this event requires a reservation deposit of 10,000 KRW.
The deposit will be charged to your credit card while issuing the mobile ticket after your application is submitted.
Reservations without confirmed reservation deposit payment may be canceled.
The paid reservation deposit will be fully refunded within 7 (seven) days after the event ends, upon confirmation of on-site participation.
However, reservation deposits for those who do not attend the event will not be refunded.

사진 영상 촬영 및 활영 동의서
(Consent Form for Photography and Video Recording)
본인은 Merz Aesthetics에서 주최,주관하는 ‘Ulthera ® GOLD STORY DAY’ 행사와 관련해 사진ㆍ영상 촬영 및 음성 녹음과 향후 편집된 영상을 아래와 같이 활용하는 것을 허락하며, 촬영된 사진ㆍ영상 및 편집본의 저작권 및 소유권이 Merz Aesthetics에 있음을 동의합니다.

I hereby grant permission to photograph and record videos and audio during the "Ulthera® GOLD STORY DAY" event, organized and hosted by Merz Aesthetics, and to use the edited videos as specified below. I agree that the copyrights and ownership of the recorded photos, videos, and edited versions belong to Merz Aesthetics.
